How do I enroll in the Aetna Pre-age 65 medical plan for Meijer early retirees?
You simply complete the Aetna application along with the mandatory Bank Draft Authorization Form (they are available at the top of this page), and mail them both to BenefitsWorkshop at the following address:

Why is payment of premiums by automatic bank draft the only option?
Aetna requires the timely payments of premiums if they are to provide you with insurance coverage. The automatic deduction of premiums from your checking or savings account
eliminates the need to mail in a check each month and helps prevent the loss of your coverage if you were to forget a payment. Automatic bank draft is the simplest way to
accomplish those goals. Payment by credit card or debit card is impractical due to the transaction fees charged by the banks, and the inability to automate the process.
